Supported Living Service For Adults Who No Longer Need Acute In-Patient Treatment Care But Require Re-Establishing Themselves In The Community
We will offer dedicated care and support for adults with mental health needs who have recently left inpatient treatment or whose mental health has declined, making independent living challenging.
Our approach will be grounded in proven methods, providing a clear path to help individuals regain their independence, manage their mental and physical health, support their recovery, and ensure they receive ongoing health and social care in the community. We assist individuals in sustaining their own tenancies through tailored support and progression plans.
Our home will be newly decorated and well-equipped, with regular reviews and expansions to maintain high standards. Each resident's room is thoughtfully furnished with a wardrobe, chest of drawers, medi-safe, bed, and chair. The home features two communal bathrooms, a fully equipped kitchen with individual lockable cupboards for personal belongings, a dining table for communal meals, a main lounge for relaxation and socialising, and a garden area visible from the separate staff office.
The home is designed to provide a welcoming, homely environment that combats institutionalisation while fostering supportive structures, routines, personal responsibility, and a high quality of life.

Supported Living following on from inpatient care
Eligibility
Adults aged between eighteen (18) and sixty five (65)
Adults who are entitled to section 117 after-care will be under what is known as the Care Programme Approach (CPA)
Have complex mental disorders as the primary support need, with comorbid difficulties such as but not limited to Attachment Disorder, Atypical Depression, Obsessive Compulsive Disorder, Bipolar Disorder, Personality Disorder, Depression, Schizophrenia
Currently residing in a time limited service for people with mental health support needs.
A history of long-term hospitalisation, a number of admissions to hospital or at risk of becoming institutionalised
Currently residing in a time limited service for people with mental health support needs.
Financial support needs, paying bills, shopping, budgeting, money management and accessing benefits including direct payments and personal budgets
Activities of daily living support needs
Medication support needs, promoting the safe and effective use of medicines and ensuring that suitable and high quality care is provided to residents
The individual is currently in a mental health hospital and is ready for discharge.
The individual agrees to continuing a programme of rehabilitation within a residential setting
Forensic history
The person is subject to the provisions of the CPA
Vulnerable to abuse, exploitation from others or self-neglect
